When parenteral nutrition is indicated

Parenteral nutrition – intravenous delivery of nutrients through a central or peripheral line, as total nutrition or as a supplement to oral or enteral intake – is indicated when requirements cannot be met orally or enterally, particularly when enteral nutrition is contraindicated or the gut has failed.ESPEN Terminology 2017 · section 3.5.3.3; ESPEN Home PN 2020 · rec. 1, 2; ESPEN ICU 2019 · rec. 6

In critically ill patients with contraindications to oral and enteral nutrition, it should be started within 3–7 days, earlier and progressively in severe malnutrition; when enteral nutrition is not tolerated, only after reasonable strategies to improve tolerance have been tried. In surgical patients who need nutrition and cannot be fed enterally, it should be started as soon as possible.ESPEN ICU 2019 · rec. 6, 7; ESPEN ICU 2023 · no. 18; ESPEN Surgery 2021 · rec. 8

Contraindications to parenteral nutrition

According to ESPEN, there are no absolute contraindications to parenteral nutrition as a method; organ failure and metabolic disease can reduce tolerance. Contraindications for a specific product are therefore listed on each product page, taken from its SmPC.ESPEN Home PN 2020 · commentary on rec. 1; ESPEN Home PN 2023 · no. 1
